This was a good question cause I like a bunch of these but I went with the one that scared me the most when I first saw this movie. The dog thing.

Spider-head for me too.Hate spiders,especially the BIG ones made out of human heads.

I picked the Dog one because it scared the Bejesus out of me, It had the creap facter and the jump factor, I wouldn't go near a Dog for a while afterword, I'm still carefull around them. Next up was the Spider head. Followed by the Big Blar One, The different heads was just plain Freaky.

Even though the poll is long closed I too would have gone for the spiderhead. Absolutely jaw-dropping moment and totally shocking. But the other monster incarnations within the film are just as good, but the spiderhead just had that extra "something": more so if you hate spiders generally, like I do.
